Form entries are not populating in mysql database

Hi,

I am new to databases. I have a website and using a registration form in it.
The entries inserted into the form are not getting populated/updated in the mysql database.I have checked and found the database connection is ok.
Can anyone help me with at least understanding the exact issue?

What is the code you are using to update your database?

This could also be a problem with your form page as well.
