Looking for upload system

I am looking for help on making a web page that has the following element on it

1. A browse button and text box that someone can browse for a file on their system.
2. a text box that they can enter a username in.

What I want it to be able to do is when someone does the things above and click on submit, it uploads the file to a location and uses the username as the folder (eg if I put in a file called me.bic and put in the username as ironhorse is would either create a folder and name it ironhorse or find one called ironhorse and put the file in it)

I have no idea what I am doing to make this (i don't have any knowledge in this field) but I need one

P.S. I dont know if you can specify the file name type at all (only .bic files allowed)

You need to use a server-side language. Do you have one installed/enabled?

not sure what you mean but I have 2k server on it with IIS on and PHP and MySQL on it
