    Default Try #2 - jQuery Form to Form Wizard (v1.1)

    3) Describe problem: None of the info entered into the fields are sent within the email....Only the radio button info is.

    The above issue is within FF & Chrome.
    ...check out what it does with IE 8.

    talk about a non functional feature.

    Sorry for the delay in response, with the constant stream of questions it's easy for some to fall through the cracks unfortunately. To your question, are you saying the script works properly for you, but that when you try and submit the form, only the contents of the radio button gets submitted? If so this is most likely a problem with your server side form script (http://www.hickstele.com/cgi-bin/cgi...ckst/mail?John), and not the Form Wizard script. The later is a client side script that merely dynamically transforms the look of the form, nothing more or less. It should not have any effect on the form's functionality itself as far as what gets submitted. To verify this, try removing the Form Wizard script, and see whether the problem remains.
