Script: Not sure what kind I need

Hello again. I'm normally tinkering with HTML advanced type code when I need help.

This time, I'm not so sure I know what I need. I'll give it my best shot explaining.

I would like to create a page like a form. When the fields are completed. The data entered will add a link to one existing page. And the fields completed will create a new page with details about a sports players

1. So basically there will be a form
2. Adds a text link when completed to an existing page of alphabetically aligned links.
3. And it will create a new page which shows the details from the fields completed for each new player who is added to the site.

I've got the links page already made. It's basically a page full of player name text urls in alphabetical order. When each is clicked it goes to each players page.

I hope I explained it correctly, I can send the examples if somebody can help.
