1) Describe Work Request (be as detailed as possible):

I am looking at a script that does two things,
receives form data and posts it to a mysql database and emails a link that would retrieve the data and display as a page.

The form is a feedback form which when submitted and emailed the reciever of the form will be able to modify two columns and re-submit it back to the database. The form will then send a link back to the original sender and an additional text box to be included for further feedback each time it goes backwards and forwards a maximum of 4 times. It will have various drop down menus and text boxes in the original form.

I would need to add additional areas i.e. names and recipients in the form for future use.

Although I have a basic knowledge of PHP its not majorly indepth. should anyone have an idea please quote me and I will be able to supply all information required.
