Write to database from email form

I'm looking for a way to email a form to someone which will automatically update a database when sent back. Does anyone know how to accomplish this? I've seen some stuff with pdf emails but I'm not too sure. It would be a form in the email body and the responses in the form are automatically sent to a database or excel file once the email is replied to.

The best way to do that will be to host the form on web server and email the link to the user.
So the user will fill in the form online and when the form is submitted on the server side you can make it update database and/or send email...

I agree, and edit the posting script to send an email copy back to the person who submits the form when they are done.
