    1) Describe Work Request (be as detailed as possible):

    I am looking for either a website in existence or someone that can help me accomplish this task.

    I am looking for a way to easy manage, print and fill out forms electronically. I want to integrate it into a web site if possible. Basically I want to be able to enter about 20 pieces of information about a person into a database (name, phone number, address, etc). Mysql will work for my purposes. Then I want to be able to select the person and their data populate the webpage in certain spots as if a form is being filled out. On said forms I want to be able to put the fields for the name, phone number, address, etc over top of the form. That's all. I have 25 people and at least 1000 forms. Plus more forms will come so I am going to need an easy way to add and organize these forms in the future. I have been searching all day and I can't believe I have not found an online solution for this.

    No payment methods needed.

    Yes. Either pay for a service someone here might already have or pay for the code to handle that. Or maybe a joomla module?

    Are your forms set up with easily identifiable fields (e.g., does each form that asks for the person's name have the same name/id="whatever")? Are you actually submitting your forms, or are you just looking for a presentational effect?
