Looking for Simple ASP form

I'm looking for a simple ASP form including confirmation page with the following sections that will send the info to my e:mail address. Nothing fancy it's just asking users if they would like updates on modifications to the website.

Sections:-

Name:-
E:Mail Address:-
Drop Down List:- (approx 5 entries will be in the drop down menu).
and a submit button.

Currently have a bravenet form but would like to get away from that.
