chained selects at http://www.dynamicdrive.com/dynamici...menu/index.htm

I need to do a fairly intricate form but think "chained selects" can help. I need sort of a "drill down" form. here is how it would go:

user goes to form
user selects club option(7 clubs)
user selects employment option
user fills in form and hits submit
based on the club option, the form gets mailed to different email..I dont know how to set the email to change and then get sent to the proper user..any help is appreciated...
