Script: DHTML Modal window v1.1

Good morning everyone,

I’m trying to install the DHTML Modal window v1.1 onto my sales pages so I can build a list through PHPlist, (http://www.phplist.com) Can anyone Show me how to format and install the DHML pop-up onto my sales pages? I know how to make a basic form with pictures and text for the pop-up to show but I don’t know how to put it on my sales page or why the form seems to be running off the pop-up box and scrolls down to the bottom. I have remote assistance application “Team Viewer” installed so if someone could show me what to do I would be very grateful!
