File Not defined in All levels Menu Generator

I have downloaded All Levels Menu Generator. (Uber color). at http://www.dynamicdrive.com/dynamici.../generator.htm

I keep getting an error message that ddlevelsmenu.setup is not defined. I have copied all the appropriate into the subfolders as required. Did I miss something in your instructions?
