Adding controls dynamically

Hi
I need a solution for this....

" I have a html form of which it contains 5 text boxes after that a button in a row.
If i click on the button another row of 5 textboxex has to be added to the table.

How can i do this...plz help me.

I've produced something like that before. Hopefully you can adapt it.

I am also in need of dynamically adding html controls to htlm form. Could you please send me the .js and .css file. Thanks in advance.
