Recall Form Values script

Can anyone please help me to add in the correct javascript to allow the following fields to be retained. Form Input Select, Radio Buttons, Check Boxes, textareas, etc... I would greatly appreciate the addons and this would improve the dynamic ability of this great script. Thank you dynamic drive!
