Add calender functions to existing form

Hello, and thank's for looking. I am looking for some help on doing something, I have not done yet. I will pay for time - code...etc. And my requirements will be.

1) I am in the process of creating my input forms, and what I need to have added (or the code for) is the ability to display (4) instances of this:

and post it into my existing tables.

but I have too many other things in process I am doing right now, and don't have the time (or patience) to do it...and figured I would post here if someone (with experience with it) could just whip this out, a heck of a lot faster than I can...and paypal you.
