embedding a script within itself

3) Describe problem:

I am trying to get this to work within itself, ie. when the viewer chooses an option in the dropdown, another dropdown appears instead of text. Any ideas on that? I have tried copy pasting the <form> code that goes in the body within a div id for a specific option. I also tried separating the portion of the script that goes in the header into a separate javascript file that is embedded in the header of the page, but it didn't seem to help.

I know questions about multiple scripts on a page aren't generally supported, but multiples of this script are working great! Just not when I put it inside itself.
