Form Manager used with Chained Selects?

I would like to use the Form Manager script with the Chained Selects script. I can get the Form Manager to work correctly without the chained selects, and the chained selects working in spite of the Form Manager. But I can't get them to work together. I have attached the javascript code I'm using. You can find a demo of what type of aesthetic I going for at palapastructures.com/webcartformtest2.htm, and you can find my html code that I'm trying to work with at palapastructures.com/webcartformtest4.htm. Any suggestions would be greatly appreciated.

Im sure there is alot of people on here who would be able to offer solutions to the problem but dumping a few links and asking to make it work wont be a good way to get anyones attention. Please specify your problem more clearly and show us some code.
