Send an Email script... is there such a script?

I want to have a place on my site where people can send an request thru email. Like this page: http://www.geocities.com/westcoastre.../requests.html

I couldn't find any script like this on DD. Could someone tell me where it is on DD or direct me to a similar site that has it? I'm sure it's out there somewhere.

That's not a script, it is a mail form. Try Googling "mail form", with the quotes. Most of these mail forms use formail.pl or other server side scripts. To use a mail form, you can either ask your host what they have available for this sort of thing or use a third party mail host. Similar work can be done using javascript but, it is much less reliable because it depends upon the user having his/her mail program configured for use with the browser.
