looking for form that prompts for email before downloading file

Was hoping someone could point me in the right direction.
I am looking for a script that prompts the user for their email address before downloading a file.
Can someone help?

Ill make this code, but I need a few things:
Do they have to type in there email? Or can they just click cancel and download the file?
Where do you want to store this email in? A file? A database?
