looking for a script

I spent a while looking through the scripts offered here, i may have missed it, sorry if i did.. but a search didnt really bring up anything either.

I'm looking for a script that when a certain option on a drop down box is selected, a edit box is shown below it(basicly for times when a person clicks "Other" to not show the box for them to explain what "Other" means unless they actually click "other"
