Free modifiable online surveys?

Hey all,
Hope everyone is doing well! (I hope I'm placing this thread in the right spot)

So here's the deal, I want to make a basic questionnaire to gain input from users about the site. I noticed that other sites use outside survey sites to do this... Anyone know of a free survey creating site? I searched and have only been able to find free trials. I want to be able to view the results privately as well.

I want users to only be able to complete the survey once... I don't know if this option is feasible but I hope someone can help.

Eh, just learn how to use forms in html.

Interpreting them server side can be a pain, and using the mailto: method is just short of idiocy, so I'd suggest looking for a free form mailer (google those keywords) to make it easy on yourself if you'll be the only recipient. It would be better to write it in PHP/ASP/etc in the first place, but also harder, and no real need if you're ok with the standard format (and ads) of the free form mailers.
