Omnislide - Adding Form + Image to slide-in menu

First of all, congratulations on a fantastic script!!

Secondly, I am very new to programming (have not had any formal IT education) so I appreciate your patience!

I am looking to insert the 'Purchase Point' image and form script button from this page into the slide-in menu.

So the whole image, including the drop-down box and button - should slide out.

I can make the image appear in the slide in menu but cant get the button to also appear... some help would be much appreciated!!
