MYSQL User Login

I want to have a login page on my site, I wanna use some secure method unlike javascript...
Some info:
Database Name: CustomerDirectory
Table Name: Customer
Fields: user_name, password, create_date, email

I want for the user to input their username and password then go to a secret page where the user can upload a file...
I think this might require some CMS, but if its just possible to have a login without a CMS that just allows the user to log in and be able to upload a file, then thats cool *I got the file upload form... so please don't make one for me...*

You can get a basic idea on how to accomplish this by looking at the following:

After you master that, you can modify it to suit your needs/liking.
