feedback rating scrips needed

I'm looking for a script or two that will allow my website the accept user-based feedback, and then rate the feedback in a few key ways:

• specs
• i want to have a list of up to 10 items ranked 1 - 5
• i want the average results of each individual item on a separate page
• I want an overall average for all items on a separate page
• i want to be able to define the rankings based on 3 different category lists.

I have found the moseover script on this site, and would like to incorporate the overall ratings list into this script. on the page listing the results, i would also like to display the total number of votes.

one of the biggest concerns i have is the ability to take out unwanted votes, as this is going to be used for a business website, and i don't want non-customers entering data.

in case i haven't described this very well, a good example of what i hope to accomplish would be ebay's feedback. on ebay, you can go into a feedback area to enter details of a completed transaction, and the overall results are shown on the page that lists a specific user's feedback. I'd really like something similar to this for my website.

any help with this would be greatly appreciated, as I hope to finish this portion of the website this week.
