Need help to figure out where to put if possible, an email. I need this to be submitted to a certain email address when they fill out the forms. If anyone could help where to put it in the code i would apreciate it.

Well, Javascript is a client side scripting, it can't send emails.
You'll have to use server side scripting such as php or asp...
