message form/feedback form html code

im trying to create one of these forms on front page and ive checked out a gazillion pages online. everytime i try one of them, i have absoulyte NO success. is there any html code/ step by step for doing this?

every website i go to either skips little steps or doesn't work... please help me!

Also, be sure you cut and paste any code into notepad before pasting into frontpage HTML tab. Otherwise you'll get a ton of garbled crap....
