i need a lil help with a submit entry to email!

Im making a somewhat of a survey and i need a lil help with the basic ..

i had a example one but is really dificult to understand... well for me.
is for an entry form to a college. Im trying to make a page for my own college and what to put a submit to data of : First Name/Last Name/age/ and the normal stuff ,but also add the Subject of Interest/Program of Interest in a more simple way!!like it was for a college!! can anyone help me with this survey submit to and email???
