Looking for a script for a Quiz?

I have been looking for a script (simple script) for a quiz. Basically I want to ask about 25 questions, which will be multiple choice. Then you'd hit submit and it will return the number of questions you got right. It would also give you the quiz again with the correct answers for you.

I have Googled around but the ones I found I can't seem to get working. Anyone know of a simple script along those lines? Thanks
