i'm looking for something like ebay has for there feedback, i sell used parts online and i am putting together a web page and would like to have 1 feedback page. i sell on many forums thoughout the internet. any help or pointers wold be greatly appreciated

When you say:

This may limit you to what these various venues allow, unless they are all mastered by you. In which case you might want to look into a third party form that you could link to all of your content. These can be cheap or free (free is usually with ads) and good ones have an interface that is easy for most people to use (both from the set up and end user points of view). Other advantages are that you can have the form linked from anywhere (even on sites you do not master, if this is permitted) and all of the responses can go to one address used specifically for this type of mail/feedback. These third party forms usually allow for a way to identify where the feedback originated from (which site). Just Google:
