First, you would need a server side language and a database (PHP and MySQL). Then; after creating the database tables that will store the user information, create a registration form (HTML). After that, set up the PHP script to parse the information that the user has entered in the form and store it in the sql db. There are quite a few user registration scripts floating around the web as well as on here.
