Email subscription

I have the site http://www.easytraveller.gr and want to collect emails for those interested to receive our newsletter for travel related offers. Does anyone have a good looking tool (code). I plan to put it at the end of the page next to Weather. Want to open a small pop up page where to put the email details.

Download and edit the attachment below. Inside the zip are the files "index.php" and "subscribe.php". Inside subscribe.php there are two options. The first is to send the info by email, and the second is to save in a database. It is pretty well commented, so you should be able to edit it without problems.
