Looks like PHProxy to me.

Yes, that's what I thought too.

PHProxy is good, but limits it to http data and a bit of other stuff.... plugins, etc. don't work with it.
CGIproxy, I have heard, does this better.

I've been using PHProxy myself, but never needed/bothered to upgrade.

1. The title for this thread is stupd. Yes, you're posting... you need something. Use the title to describe WHAT, so we look and know what to expect (and can find the thread later and such). Plus... spell it correctly. It makes me care a lot less when you don't even use all the letters in the words.

Well, this is looking for a specific already-existing script.
I take the new board to mean more for "how do I make a form that people can enter their birthdays and it'll tell them.... etc" as opposed to a question about, for example, phpBB, etc.
