Please help I need a fill in form script

Hello, I am a newbie!

Just wondering if anyone knows of a script that will enable me to allow the user to fill in some boxes and then it will generate a letter. I am building a non profit website to help teachers and would like to include an automatic reference generater. Does anyone have an ideas?
