Photo upload and form not working, Please help - Urgent

My form with photo upload is not working the code is to much to post here and I have it posted at another forum. Can some please help me with this. I have the form, Submit page, DB page and the upload page. Here is where you can view all the code. Please help, I have no clue why its not working...

Post the error message that you are getting.

Im a newbie and im not sure of the error. I am using some third party code and its just redirecting me to the listing page and stating Operation Failed. I really need to get this working I would be willing to pay some one to re-write the code for me. I am pulling my hair out here.

Can you tell me exactly what you want to do? are you looking for an ASP code that will upload the images/files? or do you have any associated operations with it
