seeking help with FORM scripts in html page..

Hello,
I am putting a site together and have run into a roadblock - namely, I STINK AT SCRIPTING! So, I've stuffed my pride and humbly ask for your professional expertise in making my form function properly... specifics - site is not uploaded yet,. template has form with inputs for both text and 'select' fields... I would like the information to be sent via email to a designated recipient and have a pop up thank you window - that, when closed will take the visitor back to my home page... make sense? Any help OR SUGGESTIONS ARE GREATLY APPRECIATED! If there is any easier, cleaner or more simply way.. please lead the blind.
