Need Help Making Conditional form that Autofills Shopping Cart with Products

Hi All! This is my first post, and I would like to thank any and all who help me in advance, as this is a pretty tall order for someone of my current skill level.

What we would like to do is have a conditional form once filled will populate the shopping cart with an appropriate product bundle. I modified a conditional form that I had build at formassembly.com, and for the most part I have a decent understanding of php, and html, when it comes to any of the other languages, I'm still pretty much clueless.

As far as I can tell in order to populate the cart, I will need some Javascript added, however I know next to nothing when it comes to javascript. Though I am trying to give myself a crash course in JS, I would really, really, really love some help, or just a push in the right direction.
