secure log-in & file upload, backend work.

1) Describe Work Request (be as detailed as possible):

Client wants a medical site that doctors can securely log-in and upload patient files. I'm more of a front-end designer and am looking for someone who has experience developing something like this (samples would be nice).

What I need is:
An estimate of what you would charge.
How long it would take.
What platform you would use?

I'd like to hear more about what you need for this project. (e.g., Do you need a simple file uploader or a form system that uploads to a database? Do you already have (or need) a login system? What are your system security requirements? etc.) I mostly use php/mysql for project of this type.
