Form Submission - Keep html

I was wondering, and I'm sure this has been asked a million times, but i could not find it anywhere, after searching google for a week, I am trying to find a form submission script that takes almost like a "screen shot" of the form they just filled out,
this is the form that i am using

just wonder how i can email it to the customer so it looks just like what they filled out!
