new reference page on submit

We're looking for a script (perl or php or javascript we're guessing) that will take a user's check box selection(s) from a form (or something like) and create a new page (by selecting submit) with just those items selected showing on the new page - to be used as a training reference for the user and user's manager.

It doesn't need to be fancy (we don't want it database driven if we can help it, our tech support is very low). It's for training compliance.

I would probaby involve a combination of JavaScript and some server side language such as PHP. But regardless, I don't think you'd find an out of the box script that does this, especially since the details will be very specific. Hire a coder perhaps?
