Online Instant Price Quote Script

I want a script (javascript, cgi, php... whatever) so a user can fill in numbers, check a few boxes, select from a few drop down menus, and they will come up with a customized price quote for their situation. I found something similar here:

Is there a script around that will let me copy and paste and edit the values for it?

I need a drop down like on this page:

The different zips will have different values associated with them.

Then there will be check boxes for services with text boxes beside to input the quantity of each.

I want the minimum charge box to be equal to $15 more than the service call value.
