strange issue

I decided to remove this from my site. I removed the particular code from every page, re-uploaded them all, deleted the favicon gif file, cleared cache, cookies, restarted PC and the icon is still in the favorites.

What have I done wrong here?

The browser often will cache the favicon gif, so it may take a few days before it's removed from the favorites menu. This is especially true in IE.
