Updating Database After Paypal Confirmation

Hello all,
I'm a relative newbie here, I program in other languages and dabble in php and mysql.
I have everything working correctly EXCEPT I have no idea where to properly place the call to update my database so that people only get their item after paying for it.

I would greatly appreciate some help figuring this out, as I've been trying to fix it for nearly 2 days and not having very much luck myself.

Are you using PayPal's IPN or PDT? Have you already looked at their code examples?
