E-mail Form Layout and Boxes

Hi I am wanting to create a form with question ie Time of Wedding, Time required to be ready, and other such question and would like it to be sent as a e-mail? How do I go about making such a form?

I have this e-mail a friend on my site but not sure if it can be edited for this purpose or do I need to start from scratch?

How do you want to edit it?

Add some more input fields,or use css to change the way it looks?

Although you could probably adapt your existing html and php process script with a bit of guidance, it will probably be easier to use an online generator to build a brand new one. This form builder seems like it would suits your needs and generates the HTML and php processing scripts for you: http://phpformgen.sourceforge.net/new_demo/phpformgen/index.php

Hi Travlast,
I am using Joomla 3.0 for my site and using a component named 'Chronoform'. With that you can easily do this sort of job. Though, I am new in web site building and don't know much, but this might help u.

If anyone here knows much about 'Chronoform' component for Joomla, please buzzz. Though it's very easy to understand and use, still as a beginner, I am facing many problems with that, specially for coding and database interaction.

Hi everyone! Been busy working no time to play!!

Ok up-date:

I have tried the on-line http://phpformgen.sourceforge.net/ne...mgen/index.php (thanks Beverleyh) It was great except it would not down load for me it said Safari can’t open the file “form299515-4.zip” because “form299515-4.zip” is still being downloaded. and underneath decompression failed? Maybe my zip needs updating?? I'm on a mac?

So then I went to the next:
Dev Ingredients and have now a successful email form (which is off-line as back-up) and now the modified version of it isn't working properly with the new fields!
The two new area's / fields: wedding time and required time of finish, now except the info as before they would copy from message box to those fields and over write them on sending so I rename them, but it still doesn't show in e-mail along with there e-mail address? Sorry ajfmrf I didn't get change to answer, I want to be able to ask clients a list of requirements which will help the team on the day like time and hair length and type etc, I only need this info to come though in an e-mail so I can forward it to the team, it would help if it was like question and answer with both showing! I am using dreamweaver cs3.

Ideally I'd like to have a booking and payment system which I can add question form to but is all part of the same process so that the client doesn't have to fill the same information over and over! The booking system I using is good but does offer payment yet but I'm told soon!
I have ecommerce site, so I can make services like product's but they have to do things twice!! Plus the form!! I know it's only 3 stages but I want to simplify it!!! http://www.globalglam.co.uk/payment&bookings.php
