Form Help Wanted

Ive spent today designing and building it with Frontpage & Dreamweaver.

Ive used Response-o-matic.com to send all the Form entries to me in an Email

Whats everyone think of my Form.

Have made it so people can complete it to use my Service.

well, you need to add a script in there that will make people fill in the required info. i clicked submit to test it without filling in everything, and it accepted it with a blank form. also, remover the table border.

Firstly, using Comic Sans MSmeans your site will look different on platforms on which this proprietary font is not installed, and means that your site looks unprofessional at best on those on which it is.
Although, apparently, it makes it easier for dyslexic people to read.
Secondly, your email address is open to email-harvesting spam-bots.

Both the above points are valid also.
