PHP Photo Album script v2.11 missing file?

Hi,
Try as I might I can't get this script to work.
I noticed on the set up page it says 4 external files required to make the script work. I can only see 3? Anyone know if I am missing something.

As I remember it the 4'th file just refers to the loading image, so you shouldn't be missing anything. Do you see anything at all on the page or is it completely blank ?. Are the files on a server online ?
