I need to get some info into a form and then submit it. any help

You need to give a lot more information.
You can use PHP and MySQL to do this, in addition to an HTML form.
What do you need help with?
Start here, for example:

Start learning some basic PHP and some basic MySQL. Then after that when you have some more information, we may be able to help.


Alternatively if this sounds like too much work you can setup a form's action to go to a "free form mailer" (see google) and get the information emailed to you.
