Hey i'm looking for a script to put on my PHP website that will allow people to go there fill info out to join my clan. I don't have any code ATM but i was reading a website and i viewed source code and told me to go here.

That is what i'm basicly looking for, but i want more stuff to it like Reason for wanting to join, and also "Games you play?". Mabye some more like age, location , and E-mail. If someone could help me with this script i would be very happy. :)
