Looking for a script

Not sure if there is a script for this ?

A lot of sites have a page where there is a lot of free products that you can sign up for just by checking the box beside the item. At the end of the page you just click submit button and you have signed up for the items. Is this a script or just coding? Im a newbie!

The checkboxes themselves are HTML; the signing-up script is server-side.

Do you know where I can find that kind of script/program? Or do I need a programer to make it. I dont have the funds for the later. :(
