1) Script Title:
Jason's Date Input Calendar

3) Describe problem:
The Jason's Date Input Calendar Script was working fine on a server while testing.
Changing the server (godaddy.com) the formfields for the date and the calendar doesn't appear any more.
Is it a server problem? Are there any preferences to change? Where do I have to search?
