Uploading script- Easy uploader

Hello, I really need a script like easy uploader http://www.phpscriptz.net/. Basically i need a script that uploads pdfs, jpegs, gif etc... but i want the form to be custom, as in i want to fill in the categories and im not sure how to create it my self, so im looking for something pre packaged.
