need to design Online Form to PDF document

Here is exacly what i want

In my college i have been given a task of designing and building an online form which will run off of a server but when its filled out one copy is saved in a database and other is generated apon request as a pdf for emailing or printing out

i am not sure how to go about this can someone set me on the right path

yea i also forgot to mention is that the pdf in question will be a pre set letterhead and all this i would want coded in php
